Form 201A lists your medal, award and training authorizations. To obtain Form 201A which you must mail or email (.pdf only... no jpg, gif or picture) a copy of your Report of Separation (DD-214) to The War Library. Include your full email address. Very soon after sending your Report of Separation your Form 201A will be emailed to you. You should immediately print and staple your 201A Form to the back of your DD-214. No. Blank documents cannot be obtained from The War Library. Each replacement MUST display the official recipient's name, rank and serial number.
No. Whether you changed your name voluntarily, got married or divorced since your discharge, the name that must appear on your Display is the name you used during your military service and displayed on your Report of Separation.

Yes. Your CDrom will arrive useable on any PC CD-Rom drive. (Some dual DVD-CDrom drives may not be compatible if incorrectly configured.) Your CDrom can be duplicated, its files copied, and your Display Recognitions can be repeatedly printed.
All hardwear (medals, ribbons, badges, etc.,) is manufactured here in the United States for the US government, which are sold in Base/Post Exchanges on US military installations.
Yes. All Display Recognitions are available in wallet size.
Yes, in limited cases some Display Recognitions that are not normally produced for inventory due to their neglible demand (not expected to be requested by more than one person) can be created for recipients whose authorization is verified. The fee for Special Order Display Recognitions is $45.00 plus USPS shipping. or $45.00 for full-color emailed pdf. Standard fees will apply when Special Order Display Recognitions are reqested by more than one authorized recipient upon its creation.

No. Citations and Display Recognitions are two separate documents. Although some military units will exceed Congressional mandate by printing part or full Citation text on a military-issued Display Recognition, that procedure is prohibited. Citation text cannot be placed on a replacement Display Recognition.
No. The War Library must obtain a copy of your original DD-214 DIRECTLY from The Record Center in St. Louis, MO.
Yes. Burial with military hardware is a worldwide tradition that spans countless past generations.
The United States is believed to be the only Nation that does not have unit medals to match its unit ribbons because the U.S. military and Congress has determined that while a medal reflects personal or individual contribution, unit contribution must be shared with others in the unit.
Yes. By law there is no limit to the number of each medal you may possess or give to a relative in your name. However, although it is not required, you should also provide your relative with a photocopy of your Report of Separation and/or Form 201A.
No. The War Library is not authorized to award medals. You must apply to your Awards Branch for authorization. Or, you may apply to the Records Center to determine if authorization was previously issued.
2/3. Any personalized item (certificate, wallet card, etc) that is personalized with a name and/or SN/PSN shipped or emailed is a labor-intensive product prepared exclusively for the singular individual named on the document which cannot be re-sold to someone else cannot be returned or refunded. There is no time limit in identifying a valid error on a certificate to request a corrected Certificate be re-issued.
3/3. Of course, any Library-committed typographical errors on a certificate or card will be immediately corrected and sent to you without cost. However, any errors committed by the recipient will be replaced and only a USPS shipping cost will apply. There will be no document correction cost.
If you are Retired or Discharged and no longer on Active, Reserve or Guard duty you may wear any foreign or Commemorative ribbon on your Honor Guard or former US uniform. See the Order of US Military Medal Precedence for proper ribbon arrangement.
Yes. The fee to create a new Display Recognition that has never been requested by anyone before, and is expected by The War Library to be rarely requested by others in the future, is $45.00. However, anyone requesting this Display Recognition after it has been produced for an initial requester will pay the standard Display Recognition fee.
